Openings:
We currently have 1, male, bed open in our 47th street
location and 3 beds, male or female, open in our 101st
street location. These individuals can be ambulatory or
non-ambulatory.
Residential Living Services
Grace
Supportive Living Services, LLC has licensed adult family homes
locations
available for indidivuals that require 24 hour supervision and support services.
All
the homes are wheel chair accessible and feature 2 ramped exits and roll in
showers. The homes all have furnished private bedrooms including television with
cable. Residents receive assistance with all support services and personal care
needs.
In Home Personal Care Services
Grace
Personal Care Services, LLC offers in-home assistance with activities of daily
living such as bathing, dressing, grooming, meal prep, and light housekeeping.
We have a dedicated staff of Personal Care Workers available to provide care to
individuals in their own home. Upon client request, we may also employ qualified
family members or friends to provide this service. If you have WI medicaid
insurance and meet the medical necessity guidelines, this service may be covered
with no expense to the client.
Supportive Home Care Services
Grace Supportive Living Services, LLC provides Supportive
Home Care Services to individuals that do not require direct
(hands-on) personal care assistance. Supportive home care
tasks may include shopping, meal planning and preparation,
and other household maintenance activities.
Companion Services
Companion staff may assist individuals in some areas of daily living as well as
provide for safe enjoyable socialization opportunities. Clients can remain
active in whatever activity they choose with a sense of security in regards to
their health and safety.
Hospital and Nursing Home Discharge Planning Assistance
Grace Supportive Living Services, LLC can assist skilled facility staff with
locating suitable community based living arrangements. The goal is to make sure
the client has the necessary supports in place to transition safely back in the
community.
